PHP and Ms Sql expert needed

I simply cannot figure out why my mail server stopped sending email via PHPMailer. I have upgraded to the latest version and PHP 7.0 and I've gotten to a point where the script says it is sending the mail, but nothing is received. I need HTML-base emails to send.

I also have an issue where an array contains all the form input data I need, but one column is not being inserted for some unknown reason. I need immediate assistance getting my form to mail working again QUCIKLY, and perhaps some help as to why this one field is not being added to the database from the array (where it does exist).

This is a sports website and I do most of my own coding, but I am not an expert - and this comes out of my own pocket as I am not getting directly paid for this.

Anyone have a few minutes to help me get this cleared up?
