Paper form converting to simple php web form

I need the attached form creating as a simple php form. When the user submits the form an email needs to be created in the format:
field1name=answer
field2name=answer
etc
and emailed to a given email address for further processing.
The first field on the form is a reference number, I need the ability to pass this into the form via the url - for example a querystring parameter of ref=123 would pre-populate that field with 123 and disable it for entry, if no ref is passed then it should be a standard text field.
Ideally when the form loads it would just have the ability to enter one trustee and one beneficiary (plus the additional wishes). Under the trustee fields it would then have a link to "add another trustee" which would display the fields to add a second, thrird, fourth etc And the same for beneficiaries.
The only catch is that I need this doing quickly, it is something we would normally do in house but we're snowed under and the timescales are tight - by the end of the week!
