Offline site survey on ipad software with sync to office

Description of every module: This is a revision of my other job posted, I feel it was too restrictive to ask for a FileMaker solution only, particularly for image import and annotation.

We create site surveys for water damage and use iPads for engineers using an app called report monster. This allows us to take photographs and annotate these with a brief description and we have a couple of stock forms that it can include for site details. The problems we have with this (and I have repeatedly asked Filu the developer for the last 2 years for a custom build without even a reply) is:

Everything has to be added manually when each report is effectively the same (cover sheet, client details)

Each page cannot have more than 4 lines of text and there is no option for a text page, only a box with coloured text

We cannot add other pages (Terms, Risk assessments etc.)

Each report is only exported in PDF form, therefore a report cannot be easily edited without access to the original iPad or breaking the PDF.
Description of requirements/functionality: This should work on iPads and be functional OFFLINE (this is a must)

Each report should automatically have a cover page and client/site details page similar to the attached version.

Reports should be able to import photographs and add annotation in the form of shapes and arrows/lines.

There should be a customer signature area (to accept T&C's)

Each report would have a number of pages automatically added to the end that will include T&C's

Reports need to sync once online and should be editable from either a MAC/PC or just another iPad.
