JQuery Form

General information for the website: magento 1.9 site

Description of every page/module: I want a jQuery form for calculating how much wallpaper is needed to cover a wall.
Important this website already uses jQuery v1.10.2

The form must be dynamic and react to user inputs to gather all required information.

Outline of form design
"Do you prefer to use Metres or feet"
checkbox - metres checkbox feet

"How many walls do you need to cover"
drop down box - 0 to 10

"How many ceilings do you need to cover"
drop down box - 0 to 10

Depending on the answers to the above questions a number of input rows will be generated using the correct units of measurement.

e.g. If the user had select metres and 3 walls
Length of wall 1 __ m __cm
Height of wall 1 __ m __cm
Length of wall 2 __ m __cm
Height of wall 2 __ m __cm
Length of wall 3 __ m __cm
Height of wall 3__ m __cm

Would you like to subtract any areas for doors and windows
Door 1 Height 2 m 50cm
Door 1 Width 2m 0cm

Window 1 Height 2m 0cm
Window 1 Width 4m 0cm

Default wastage 10%
You need a total area of Xm using which will require x rolls of your selected wallpaper
