Java program - installer

I have a Java program that implements a simple proxy server.
It consists of one class file (some more may come to it later) and embedded JRE.
I need to pack it into a standard windows installer. It should install the programs as a windows service.
The service should be done the standard way that allows Windows to communicate with the service standardly, showing its status, starting and stopping the service.
Some piece of code and a library may need to be added to the program.
The installer should be able to install and uninstall the service correctly
so that the uninstallation works the standard way, triggered from the Windows Applications window.
The whole thing must be reasonably configurable so that I can add: app name, company name & logo, installation path, source code. Work on windows & windows server.
