I need to convert a form into PHP Web form with database and upload image from webcam

I need to convert a PDF / World form into PHP Web Form with database and upload image from webcam.
I have a PDF form (simple) that needs to be converted into PHP form keeping data into a database (could be MSQL) and have a "UPLOAD PICTURE field" in the form to take image from webcam.
This will be a web form with 20-30 users (each one has their own user / password - Frontend not public) to be able to open the form and fill out.
A backend (ADMIN) should be needed in order to see all the forms together + pictures taken from webcam from all the users (20-30).
Ideally this will be installed under our domain.com/forms where first you need to type user + password to have access to the form.
The form will have individual number each time a new form is create or summit the script should generate another unique number.
The data from the form should be also possible to be exported/ sent via webservice to another application.
