I need an Excel sheet recreated for a website

I have a single Excel sheet with a few input boxes which generate a few fairly simple calculations. I need this functionality to be recreated using html and javascript.

More details about the work, since the proposals I've received have a very large range: The calculation is based around bookmaker bets. Specifically, the user enters their 'back' bet, and the formula works out their other information. I have attached my original Excel, plus a PDF which shows the formula.
I have converted the code to Javascript using http://www.spreadsheetconverter.com/, and this works fine, but I need stand-alone or 'clean' code, with no references to other sites.
My hosting site has a restriction with hosting files so I can't create folders for the assets (css and js files), so ideally all code is within one file I can cut and paste into the source code. However, I have been able to upload .js files and reference them as if they were for example files in the images library.
