I need a website designer and programmer to develop registration page

Description of requirements/features: Hi, I am looking for website expert to develop and design, Multiple country Registration Form similar to existing country "Australia" registration form, Same registration process required for below countries- Canada, Hong Kong, Ireland, New Zealand, Singapore, United Kingdom, United States.

Canada, Hong Kong, Ireland, New Zealand, Singapore, United Kingdom, United States,

1. Each country should have 2 registration form - 1x for owner 1x Seeker
2. All pages should be in PHP and required to connect with existing website.
3. The design of all registration page could be different but it should match the country specific fields.
4. All registration form show country specific fields only .. example : Google API address ( only show specific to country not while world).

All other information shared post job assignment.

NOTE : this is fixed price Job, if you able to complete the Job in that price, then only for this job, otherwise , thanks for your time and viewing. Only 10-15% increment is possible, not more than that.
