I need a cms driven web site which produces completed document templates

Need a website built where customers can fill in a "form style" questionnaire which will produce a legal document (think of it as a mail merge template) and can then be saved/printed or edited in a "my account " area once purchased.

There is also the option for customers to choose one of 2 legal documents (very similar templates)

There are fields that must be mandatory and there will also be a few validation fields.

Site must be created in PHP to be added on Linux servers.

Please note that html and design already complete. The work should realistically take no longer than 6 weeks to complete.
