I ned a simple voting form.

General information for the website: Sample Voting Site
Description of requirements/features: I am designing a site where people can vote daily on their favorite products. I need a voting script like the example below so that people can vote every 12 hours. Once they vote. I need their info written to a text file on m server and have it say
"Thank you for voting! You may vote again in 12 hours!"

I need to be able to cut and paste this script into all new product pages that I add.

Please look at the voting page below that someone wrote for me. I need the SAME type of voting script that I can use on several pages. I do not have a big budget. So I will probably go with the lowest bid.
