I need a freight class calculator created for my website similar to http://www.freightclasscalc.com/calculator.php (please also see http://www.freightclasscalc.com/info.php)

I'm currently building a site and will have modal popup with this calculator in. I just need to php/java code to paste into the modal and any include files to reference.
