Form in EXCEL to Data VIEW and Data ENTRY

Task: We have a few worksheets that we manually update in EXCEL.
Whilst it's very easy to use the in-built form per worksheet, we would like to combine these all into one.
A way to simply VIEW the information and CREATE new records.
The data is relationship-based. Something simple and functional does not have to look great.
MUST be in EXCEL, using Visual Basic function in EXCEL so we can change/edit if we need to in the future.
