Form Coding Job To Create A Beautiful, Functional Web form with Pricing Algorithm

I have a successful website specialising in Electric Guitars that needs a professional & functional web form built on the sites core function of providing customers with an estimated price for their item. We really needed an automated way of replying to customers who would like to get a price estimation of their guitars sale price.
A previous coder has almost completed the work but was unable to complete the final stage for personal reasons. I need a coder to complete the final phase of this web form and ensure it is fully functional
The site is built with Weebly, and any code will need to make use of custom embed code which is what I'm hoping to use for the following element.
The website currently provides quotes to people selling second hand products to the website,
Essentially in a nutshell. The customer fills in the details and at the end the system or final code provides them an instant estimate, emails them a confirmation from the site email and forwards an email to the website administrator with details of all data collected and the price quoted (all emails have been written and just require formatting). A few additional features I have requested is that if a form is left Semi completed and the user backs out a copy of what was completed is still forwarded to the web administrator so that we can follow up customers who don't complete. There is also an algorithm i have designed that we will need to ensure functions correctly for the pricing estimates to be accurate.
I have all the details for the pricing algorithm which may need to be tweaked from the previous coders work
I have all the details on precisely how each form section should function on an easy to read workflow diagram
I also have all the template emails (which are sent out when the form is completed) completed and just require appropriate formatting and inclusion in the webform.
I would imagine there will be questions so do feel free to ask. As the majority of the code has been completed (design/data entry/untested algorithm & pricing data) the final stage of the form was to send an automatic email. If you would like the job please factor in that we will need to include foreseeable beta testing inevitable tweaks so that this form functions fantastically well and is easy to use.

Description of requirements/features: The form should be able to do the following:
1) Creation of the web form is completed using the HTML embed snippet on Weebly
2) Form must be responsive and functional on Mobile/Tablet/Desktop (all browsers)
3) Form must be designed using the same font/styling as the current website
The form should allow customers visiting the website to input their data as prescribed in the workflow diagram (which i can provide) please take a look at the attached document.
4) Form must allow customers to input all data in an easy to use form and split into easy to digest sections. There are 4 distinct sections for data entry and 2 final pages. 1 Contact Information. 2 Guitar Options. 3. Damage Options. 4. Shipping & Collection 5. Price Estimation with Accept + Decline options 6. Thank you message
5) Once form is completed an algorithm based on a price list I provide you activates and provides an estimated price for the customer with the option to Accept or Decline (Section 5 of web form).
6) Once estimated price is accepted or declined a prescribed email will be sent to both the web administrator and the customer from the sellmyjem website confirming price and providing a few further details. I will provide this email.
Additional Features
1) If a customer backs out of the form early a data dump of all completed information is sent to the web administrator
2) Pricing algorithm is fairly straight forward but requires 1 advanced feature. When selecting damage options the first damage option applies a full deduction. If a second damage option is selected a deduction is only taken at 50% of the price deduction that provided in the pricing list. If a 3rd damage option is selected a deduction is only taken at 75% of the price deduction thats provided in the pricing list. Any further damage options selected are at a 75% deduction from the price listing in the pricing list
CMS and Admin requirements: Weebly is the web design interface. I am not a coder myself so please ask anything that you need to know
