Fixing PHP MySQL connection issue

Hello, I need to connect my HTML search form with MySQL database (1 table with 6 columns). I already have the PHP code but it doesn't show any results. It is very simple, but I have no time for searching the mistake. The are 3 different search forms (dropdown menus) in HTML code and user needs to choose only one of them.
I need help ASAP. Today will be perfect if it is possible.
