Database Product Search Form for website

We're a web design company and need someone with good knowledge of PHP, mySQL and Joomla for a small project.

I have a mySQL DB with 100000 parts.

We need a form that will search through the database once a user enters a part number and return a number of parts matching it, then the user needs to select the tick boxes for the part they want, fill in their contact details and submit this form by email. This form will sit within a Joomla website which we are currently building (we're designers not developers).

The CSV which I will build the mySQL database from starts like this:

Part Number,Quantity,Description,Condition

We have RSForm PRO to use

We know that it should only take around 4 hours to complete.
