Creation of table in Excel from data

Hi,

I'd like someone to take the information as shown in the "Input" tab and make it look like the "Output" tab in the attached excel.

The cells in yellow document how you map from input to output.

I will update the input data weekly, i'd like to be able to copy it into the input sheet, click a button and have the output sheet populated. The remaining data in cells in the Output tab are example data.

If you can smarten up the output tab at the same time and make it look more professional, that'd be great.

Hoping it's quite straightforward for a VBA genius!
