Creating an online data entry form using PHP / MySQL

We require 2 very simple online data entry forms, using MYSQL / PHP. There would need to be a drop down list with the appropriate columns.

The forms should then produce some very simple reports based on the information entered in the forms. The website is already set up with a user login area using wordpress and would need to be secure.

We require this completed ASAP and also have lots of future development work using PHP / MySql
