Create small reminder programme for PC

I need someone to create a small program for me. It could probably be done in Excel or Access but I’m not unduly bothered.
I will tell you what it needs to do.

We have 20 vehicles or so. We need a small program to remind us when the road tax and MOT tests are due for each different vehicle.

We need a one line entry on a spreadsheet for each vehicle which will have:
manufacturer,
model of vehicle,
registration number,
colour,
date when the MOT test is due and
date when the road tax is due.
reminder email address (maximum of 2 for each entry)
names of persons to be reminded
number of reminder emails required) and number of days before expiry date
number of date expired emails required

Each vehicle has road tax expiring on a particular date and MOT road test expiring on another date. The dates are different. We need reminding of those dates before and warned after the date.

I have put a fixed price which is what the job is worth to us

Please don’t send me an email which just says “I can do this”.

I need to know exactly what is being proposed and to see examples of stuff that’s been created by you before.
