Create PHP Library that generates PDF documents from data set. (Mysql)

We would like a custom PHP Library that can generate PDF documents from a dataset passed from a MySQL database. The Library should be able to meet the functionality below as a minimum requirment.

Below is the items to be covered,
1. Pass and render table content, and table styling etc.
2. Place images on specific locations on the page i.e Top-left, Top-center, top-right, bottom-left, bottom-center, bottom-right etc.
3. Render images on document (apply postioning too)
4. Render html form elements with states (checkboxes, radios) i.e. checked/unchecked status different colours depending on state
5. Have the Document structured as having Header, Heading/Subject, Body Content and Footer.
This should allow us to place items on the document by simply specifying the section where to place it.

6 - Create a calendar view for a month/ a week with a selected date
7 - Generate simple graphs - pie chart, bar chart
