Description of requirements/features: - I need a standalone html file which can be run on a mobile in a browser without internet connection.
- I want to allow users to answer questions in a 'survey' fashion - using basic radio buttons (e.g. 5pt scale).
- Once a user has input answers I want the form to return what 'group' a person is classified in based on their answers.
- Attached is an illustration
Extra notes: This site gives an example of scale questions, e.g. go from strongly agree to strongly disagree
