Convert excel version of survey to online survey

I need to have a link to a survey where pet parents can complete a questionnaire. The results of the survey will be provided to a Ph. D. nutritionist to conduct a virtual consultation.

Just some additional color, pet parents will go to www.petabolix.com to get a pet food recommendation. If the pet parent is interested in a direct consultation with a nutritionist, they will complete this questionaire so the nutritionist will have additional information for the consultation.

The first phase is to convert the Excel document to maybe a Survey Monkey or something usable.
