Car number plates preview

I need to have a form that can generate a preview for UK car number plates.
The preview will be a HTML5 canvas or can be done using PHP if you prefer.

The following form fields will be selected / entered by the user as they are generating the preview;

1. Number plate (text field)
2. Size (drop down with sizes)
3. Front, Rear or Both (radio button, and if both it will show both in the preview)
4. Text style (drop down with a selection, of standard, 3D or 4D plate. The 4D plates will also have a neon option 'border colour to the text')
5. Badge (drop down with a selection of badge images which will be placed on the plate for the preview)
6. Plate border (drop down of colours)
7. Slogan text (text field which will appear under the plate number)

Very important thing to note, some plates are big and others are small. There is also the Jaguar size which has a different design from the rest.
