Build a website based around a Leaflet map

I need a designer with experience using Leaflet to build a one page website based around a map that people can add pins to via a form that would then come to me for approval to be added to the map. The pins would each have a pop-up label containing a link (as submitted on the form). The website would operate in a similar way to toppletheracists.org (see attached image).

The page would also have a form to submit your email address and location to join a mailing list and receive a daily email (notifying of all new pins that have been added that day).
