Basic VBA Form and excel update

I am looking for a basic form set up in Excel using VBA to track a few key things. It should track:
- Time and date stamp (to the second) when form is first opened
- User name field (to be blank on first opening but return same name in following openings)
- Drop down list of Processes (Process A, B etc)
- Drop down list of outcomes (Complete, Stopped)
- Drop down list of a third option (did computer go slow Yes or No)
- A start and stop time to click to time how long the computer went slow for
- A submit button to submit info, take a second time and date stamp and send results to a central table to be stored without overwritting each other
