Auto-fill form values based on URL querystring

I need a form built that will be placed on a Wordpress page which autocompletes a hidden field based on the url .
e.g.
URL = mysite.com/page.php?location = 1234
The form would autocomplete the location field with the value 1234

The form will contain 5 fields
location (Hidden autocomplete based on URL querystriing
date (Hidden default = today)
Name
email address
telephone number
On Submit, the form should insert to data in a database.
