A PHP Ajax form or any other PHP combination to upload 5 images on submit

I have a website for advertising cars. I want the advertiser to fill in the details of the car, upload 5 pictures of the car and their contact. I am having challenges getting code for uploading multiple images on submit. If possible, I'd like to have a preview of the image after browsing for it before uploading.
