General information for the app: App for completion of a form
Kind of development: New app from scratch
Num. of web pages/modules: 1
Description of every page/module: One page App with a form to be completed by user for Health & Safety incident reporting.
Form contains Time & Date selection, 6 fields of text, 1 drop down list (27 options) and 2 options (YES/NO style).
On completion of the form, the app will use default email profile on phone to send the data to an email address(es).
Email address(es) to be changed in a settings menu.
Description of requirements/features: App needs to be simple and have the ability to take the inputted data and send via email (using the phone's default email account).
Specific technologies required: All users use Windows Lumia 520 phones
