Timesheet App

Want to create a basic timesheet app. We are an employment agency in New Zealand and have all different people working in different jobs every day. Just want to get rid of the paper version of our timesheet. All apps that are already around are a bit complicated for our needs as they tend to suit people that are in a regular job for the same employer where as our staff are at different jobs for different people each day for the most part. This needs to be really basic so that everyone can easily use it. Have attached a copy of our current paper version below but it basically just needs to be able to have somebody type in the client they've worked for (which may change from day to day), the type of job they did that day, their start & finish time, their breaks (if any) and for each client to be able to sign it at the end of the day. They then need to be able to send it back to us at the end of each week by text or email. Our week runs from a Monday-Sunday.
