Simple ios app to read health data and record question answers

I am looking to create a very simple ios app for my research. I just want people to be able to download it, then to be able to write their health data i.e. no. of steps, number of flights climbed, and any other data to an AWS sql database daily. Alongside this I want them to get a notification daily to answer four questions, how they slept the last night, how stressed they feel, what there mood feels like, and how productive they feel they were.

Would this be possible.
