Mobile app developer required to fix bugs and create two screens

I have existing android/iOS app developed using hybrid platform. App needs to fix bugs related to slow speed and I need to developed 2 screens.

one screen where user can schedule appointment after login. User can select date and time, number of people, select appointment type - physical or call, and appointment location. another screen is for confirmation that your appointment has been schedule message with appointment details.
