Description of requirements/functionality: We are in need of an app which allows our customers to fill in their details on a form, on an iPad, digitally sign the form.

The form will be quite simple and the design aspect will be easy to implement. The form will include name fields, date fields, dropdown boxes and a digital signature. A postcode lookup function to populate the address field would be good.

Upon completion the form then needs to be uploaded and stored on our server.

The data from the form will also need to be sent into our CRM system, Infusionsoft. This will involve creating a new contact record, or updating an existing contact record, based on the customers email address.

I am setting an open budget for this task to encourage you all to bid, however naturally the lower priced option by a developer with proven experience will be in with a better chance.
