I need a basic app-based membership card designed

Ok, techies, can you give some thought to this before you get in touch?

This is a pre-cursor to a full-fledged membership/loyalty card concept that I will look to roll out after 6-9 months. I need a basic solution for now, as time and costs are big factors.

So for the time being, it only needs to do the following:

1) create a card, similar to the attached in digital format.
2) it needs to be able to assign a name and membership number with expiry date.
3) the expiry date must be absolute, which means, the card must not be able to load or be visible after it has expired.
4) the link to download it must be usable only once, otherwise a new one generated if something goes wrong. This will prevent mis-use and unverified sharing.

For now, this is as sophisticated as it needs to get. No back end or critical infrastructure needs to be created.

Please come with a proposal if you understand the concept parameters only and can work within a finite budget.
