Develop Prototype WebApp Frontend/Backup

Hi,
I need to develop a prototype application that will be a web app utilising a MySQL back-end database. This will demonstrate how a product is bought.

This is a proof on concept and a requirements specification will be produced.

I envisage this being quite a simple application: Login screen, menu, administration, catalog browse, add new inventory item (ID, name, description, price, phone).

I would prefer that the developer is in tune with current standard and can propose a design of how the system will look (modern and fresh) - from there I can draw the screens in visio or something like that.

The database has to be relational and again I can assist.

I will do the documentation and basically I am looking for someone who knows what they are doing so that it can be done quickly and I will support.
