Coronavirus Calculator - Mortality Rate

I need android application for calculating the mortality rate from corona-virus by answering few questions (factors) like age, underlying conditions, sex and pre-existing condition. The result will resulted from a function that related to this factors factors.
I need it in one modern interface and simple.
The data are simple and available with me.
Also simple logo related to the software is required.
Second language (Arabic) is required in the software.
