Convert a word template into a tablet fillable PDF

We have a small but increasing field sales team who visit clients in their home and collect the information required to write their Will on a form.
We need them to be able to do this on an iPad then send the info back to head office. HO then need to be able to import the fields on the form into various other document templates on Excel and/or Word
We are fairly computer literate but are short on time to develop this.
