Android Patient Report App

I am attempting to write an App for field reporting of medical events, I propose to conclude about 10 layouts. I would like someone to work with me to write the back ground coding, this will be a number of text boxes, calculations

The result would be printed to a recipt type rechargeable printer

For example the front page is attached there would be a requirment to calculate age in years on this page, and possibly print details
