HTML/CSS/PHP/MySQL Customisation

I'm looking for a developer with an HTML/CSS/PHP/MySQL to work on SuiteCRM. I'm looking to build a custom module which enables users to input different data depending on the result of a drop down field.

I need the records to all be in the same list, for reporting etc - but I need to capture different data against each item. The best example I can give is a 'travel' custom module, with the user selecting the type of travel (Flight|Journey|Stay) and then inputting the detail against each item as needed.

I will then need to be able to report against all of these & create PDF documents.
