Wordpress forms and ordering system

Design simple forms to take initial contact info and then record approx 20 body measurements and allow photo uploads with the measurements to allow customers to order dance costumes online.

We are designing a simple wordpress website and your job is to design and add this functionality to the site.

We see the process as being -
1. Customer makes an enquiry including contact information and outlines dates via simple online form.
2. MDD accept the order and agrees dates
3. Customer Pays their (non refundable) deposit using their credit cards and online systems such as Paypal and Worldpay.
4. Customer then inputs all measurements and uploads their photographs of the measurements in process and any videos about the dancer via online form approx 20 measurements. (Photos illustrating the exact measurements will be supplied)
5. Customer makes choices about colours fabrics threads patterns etc via online form. Approx 10 choices photos supplied
6. That information then supplied to MDD workshop, also securely retained online.
7. Customer contact by MDD, discussion either in person on online then recieves designs and customer agrees about final design.
8. Final price and delivery calculated by MDD and communicated with the customer.
9. Customer agrees and accepts the price and delivery details.
10. Costume is then made.
11. Balance payment then required from the customer.
12. Once payment is received delivery of the costume is set in motion

This can be simplified as the client can do some of this manually but I'd be interested to know your thoughts on the best way to do this.
