Single page website + 5 page Questionaire

Job Description

Looking for a one page web site which then links to a 5 or 6 page questionnaire.
Which will have 4 or 5 questions per page.

Each question will have a yes or no answer option. If the answer is yes then another question will appear .

If the responder ticks the box their information will then be posted to the company who posted the question. Each question will need to be tagged to identify company requesting
the information.

The web page and Questionaire areas need to be easily editable.

Please respond with rough cost and expected completion time.
