Questionnaire - database job

I have a paper based questionnaire with 80 questions in it.
Each question has a simple agree/disagree response.
There are 4 categories that these responses correspond to. e.g If someone answered agree to Qs 1,5,9 it would addup their responses in category a.
These responses need to be stored in a database along with the persons name and maybe a code number.
In addition the total for each of the 4 categories needs to be emailed out.
I have the full details available for the person who is awarded the job.
