PHP and MySQL database

All of the following I require to be setup from scratch as I have no database.

I expect the coding for this project to be done using html/php and with a mysql server. The index html/php page will be a login page. Once logged in the user will face two links, one for 'Upload File', and one for 'View Data'

'Upload Data' will have a field where they can browse, and submit their CSV file (example which you've already seen). The CSV file will always be in the same format of columns and the data will go to into a database called 'Resources' inside a table called 'Datatable'. It is possible duplicate entries could be added to the table and so I want to avoid this; duplicate rows can be found by matching the first two columns (name and date columns, as only one name can do anything at a given time to the second).

The 'View Data' page should be some form submissions where I can select the date from and to (day/month/year, I'm not interested in filtering to the times of day) and once queried it will be visible to the user in a table, in the same format and layout as the CSV is, with all columns visible.

No effort is needed to make the pages visually appealing at the moment.

Finally, this project is part of a much bigger project and so I require comments in the code so I can keep track of what's going on. If this takes up more time, no problem.
