Order form

I am interested to design a database( for my employees) in this I would like them to place different request, for example if they need a meeting , or if they want to have a dinner or if they want to order some items. I would also like to include predefine text for them to choose or to give the possibility for them to draft and specify their needs.

Is this something you could design?
