Online forms to collect data.

My client requires custom built forms to be put online alongside their website holding page. The forms are designed to collect vital information from companies that will be advertising their services on the website once it has launched. Ideally the forms need to be linked to a database so that once the form has been submitted the information is stored. The data needs to be accessed in an easy to view format for the client and then when the website is ready it needs to be easily transferred into the websites database system.

All form fields and content will be provided. The timescale for this project is one week. They need to be online by Mon-Tues of next week.
