I need a PHP / MySQL script to copy records from one table to another.

Hi I need a PHP / MySQL script to copy records from one table to another.

In the old table, I have 8 columns that repeat 6x.

Example:

field_1_1
field_2_1
field_3_1
field_4_1
field_5_1
field_6_1

field_1_2
field_2_2
field_3_2
field_4_2
field_5_2
field_6_2

field_1_3
field_2_3
field_3_3
field_4_3
field_5_3
field_6_3

and so on until 6

now I want to copy these fields to a new table:

field_1
field_2
field_3
field_4
field_5
field_6

but only those that have a value. Empty fields are not copied.

The ID from the old table will be in the new FORM_ID table
Currently there are approximately 400 x 6 records in the old table.
I need a source code script, because I do not know when I will start copying and switch to a new table.

Can you do this today? How much would it cost?
