I need a MySQL Database and Application built

I need a mysql database and database application built. The PDF attachment to this post gives an in-depth detailed description of what I need designed.

Furthermore, the database application needs to be able to be successfully in embedded into a WordPress site. The database application pages do need to be mobile optimized, but mobile app development will not be necessary.

The successful bidder Will be given full access to the backend server through CPanel and my PHP admin to deploy The database to the server, as well as full back end access to the site to ensure that the database and database application has been fully deployed to the site.

Furthermore, it is desirable that the freelancer create a installer package for the database and database application files to be deployed on a new server in the future this is not a requirement, but it is desirable.
