I need a google form creating

Hi, I need someone to create me a google form, it is going to be used to manage booking people into riding clinics. It should allow people to select the time they want to have a lesson from a series of preset times. They should be allowed to select whether the times are private or shared. Only one person should be able to select a private lesson per time slot, if anyone else tries to they should be told that time is already booked. It should allow a maximum of 4 people in any one time slot. We have multiple different instructors running clinics on different days so I should be able to have one folder for 'Charlotte Thomas Clinics' and one for 'Hannah Moody Clinics'. The available timeslots will always be the same and are as follows: 12.20, 13.00, 14.00, 14.40, 15.20, 16.00, 17.00, 17.40, 18.20, 19.00
