I need a databased developing

I have created a set of data for which I want to create a tool that people can fill in a set of numbers based on 3 questions, which then comes up with an answer.

I don't want people to have access to the data just the input screen.

I'm looking initially a cost effective way to do this - this is important.

Also someone with the ability to create the algorithm that will drive the answers. Something like 3 numbers are a full match, then for the next best option of 2 numbers that match to create the answer. Or to suggest a better way of doing this.

I can share the numbers and an example of input screen once interest is shown.
