I need a Data Base that can store records with access

General information for the business: Ambulance Activity Log
Database management system (DBMS): Microsoft Access
Description of requirements/functionality: I need an Access Database which will have:

1. Form to Enter Data with button to print the case to be given to the nurse before they go.
2. Form to Search Data, by date or by region or by locality or by case ID and function to print data searched.
3. Reporting System, I choose what I want to see; for example I need to see all cases done today or by date or from xx/xx/xxxx till yy/yy/yyyy or by region or locality or by ambulance or by EAR and function to print reports.

I already did a lot of work in it but I have some things that need to be fixed for example I need the region will always show automatically when the location is selected. I need Time In - Time out to know the time taken for the ambulance to arrive. Time Out and Date is entered automatically which is already set. I also need that once the data is saved it cannot be edited by other Login who did not enter that particular case.

Extra notes: The work is already done all I need is settings etc which I am green at so the budget is only Euro50. I am doing this voluntary to the hospital I work in.
