Excel Sheet

I want to create an Excel Sheet so I can monitor the performance of my team. My team are handling customer enquiries and generating sales and I want to see how successfully the convert these enquiries. There maybe a few ways to do this but I was thinking an excel sheet (If you have a different way please let me know in your proposal)

Sheet 1.
Name of enquiry
staff memeber name
Enquiry date
Enquiry source
Service they want
Appt attended - yes/no
Booked with dentist yes/no
Treatment started yes/no
Comments


Sheet2. Monthly conversion stats
Number of enquiries
Number of enquiries attended
Number of patients seen that see the dentist
Number that see dentist and start treatment.
Treatment value

On sheet 2 it should also have lifetime conversion.
The conversion should be of whole team and also be able to segment by staff member. So i can tell which staff member is performing better.

It might also be useful to be able to segment the data by service also. This is less important, but it might show that we are better at converting.One type of treatment over another
