Database to enter survey results

I need a simple database that is scalable and can hold results to numerous surveys done on behalf of any number of different companies. The same respondents may fill out more than one survey and each survey may have a few of the same questions. I need to be able to query information by respondent as well as by answers. for example: x number of people answered x to question x... or mrs. x answered x from survey x and answered x from survey x. or 20 out of 30 respondents answered x to question x in surveys x,y,and z etc... can you help?
